CPT 88271, 88275, 88274 (use multipliers as necessary)

Test Details

Cancer Type

Hematologic malignancies, Lymphoma

Technology Used

FISH

Turnaround Time

3-5 days

Use

Follicular lymphoma, Large B-cell lymphoma

Special Instructions

If aspiration syringe requires internal pre-coating to prevent clotting, diluted sodium heparin (green-top) tube should be utilized. Do not send syringes. 2-3 mL aseptically aspirated in sodium heparin (green-top) tube. Cautionary Note: Lithium heparin also comes in green-top collection tubes. Do not collect in lithium heparin.

Methodology

Fluorescence in situ hybridization (FISH)

Specimen Requirements

Information on collection, storage, and volume

Specimen

Bone marrow aspirate (preferred); whole blood (acceptable for limited tests); smear; slides; formalin-fixed paraffin embedded (FFPE) tissue block

Volume

3 mL bone marrow; 10 mL blood (adult), 5 mL blood (pediatric); fixed-cell pellet4-5 unstained slides cut to 4-5 microns thick, air-dried overnight; 0.2 cm x 0.2 cm x 0.2 cm tissue.

Container

Green-top (heparin) tube (optimal) or lavender-top (EDTA) tube

Storage Instructions

Refrigerate bone marrow and blood specimens; store slides and tissues at room temperature.

Causes for Rejection

Broken or dirty slides; excessive cellular debris or stained slides; frozen specimen; decalcified specimens. Do not collect in lithium heparin.

Collection

Refrigerate specimen. Use cold pack for transport. Be sure cold pack is not in direct contact with specimen during transport. All specimens should be shipped to Integrated Oncology on the day of collection. Delay in shipping may compromise cell viability and results. Air dry slides; Send in protective case to prevent damage. Tissue should be well-fixed in formalin. Do not decalcify. Store tissue specimens at room temperature (20-23.5° C). Use cold pack for transport. Be sure cold pack is not in direct contact with specimen during transport.
